Abstract

Purpose

The purpose of this paper is to review the history and current state of provision of homeless medical respite services in the UK, drawing first on the international context. The paper then articulates the need for medical respite services in the UK, and profiles some success stories. The paper then outlines the considerable challenges that currently exist in the UK, considers why some other services have failed and proffers some solutions.

Design/methodology/approach

The paper is primarily a literature review, but also offers original analysis of data and interviews, and presents new ideas from the authors. All authors have considerable experience of assessing the need for and delivering homeless medical respite services.

Findings

The paper builds on previous published information regarding need, and articulates the human rights argument for commissioning care. The paper also discusses the current complex commissioning arena, and suggests solutions.

Research limitations/implications

The literature review was not a systematic review, but was conducted by authors with considerable experience in the field. Patient data quoted are on two limited cohorts of patients, but broadly relevant. Interviews with stakeholders regarding medical respite challenges have been fairly extensive, but may not be comprehensive.

Practical implications

This paper will support those who are thinking of undertaking a needs assessment for medical respite, or commissioning a new medical respite service, to understand the key issues involved.

Social implications

This paper challenges the existing status quo regarding the need for a “cost-saving” rationale to set up these services.

Originality/value

This paper aims to be the definitive paper for anyone wishing to get an overview of this topic.

Abstract

This paper investigates the international market research activities of 96 successful small and medium‐sized exporters. The research focuses upon the types of information considered most important for decision making and the various sources that these enterprises use to obtain the information. The empirical work suggests successful firms put much greater emphasis on the information required for long‐term strategic planning rather than the day to day operations of the business. The vast majority of the firms used internal sources for this information. Strong statistical evidence is presented to suggest that the firms under investigation actively use the information they collect for decision making, rather than simply paying lip service to the concept of market research. This work makes an important contribution to the literature, because it represents one of very few empirical studies which provide support for the widely held belief that collection and subsequent use of market information is an important feature of successful exporters’ behaviour.

Abstract

Natural selection—survival of the fittest—is as old as life itself. Applied genetics which is purposeful in contrast to natural selection also has a long history, particularly in agriculture; it has received impetus from the more exacting demands of the food industry for animal breeds with higher lean : fat and meat : bone ratios, for crops resistant to the teeming world of parasites. Capturing the exquisite scent, the colours and form beautiful of a rose is in effect applied genetics and it has even been applied to man. For example, Frederick the Great, Emperor of Prussia, to maintain a supply of very tall men for his guards—his Prussian Guards averaged seven feet in height—ordered them to marry very tall women to produce offspring carrying the genes of great height. In recent times, however, research and experiment in genetic control, more in the nature of active interference with genetic composition, has developed sufficiently to begin yielding results. It is self‐evident that in the field of micro‐organisms, active interference or manipulations will produce greater knowledge and understanding of the gene actions than in any other field or by any other techniques. The phenomenon of “transferred drug resistance”, the multi‐factorial resistance, of a chemical nature, transferred from one species of micro‐organisms to another, from animal to human pathogens, its role in mainly intestinal pathology and the serious hazards which have arisen from it; all this has led to an intensive study of plasmids and their mode of transmission. The work of the Agricultural Research Council's biologists (reported elsewhere in this issue) in relation to nitrogen‐fixing genes and transfer from one organism able to fix nitrogen to another not previously having this ability, illustrates the extreme importance of this new field. Disease susceptibility, the inhibition of invasiveness which can be acquired by relatively “silent” micro‐organisms, a better understanding of virulence and the possible “disarming” of organisms, particularly those of particular virulence to vulnerable groups. Perhaps this is looking for too much too soon, but Escherichia coli would seem to offer more scope for genetic experiments than most; it has serotypes of much variability and viability; and its life and labours in the human intestine have assumed considerable importance in recent years. The virulence of a few of its serotypes constitute an important field in food epidemiology. Their capacity to transfer plasmids—anent transfer of drug resistance— to strains of other organisms resident in the intestines, emphasizes the need for close study, with safeguards.

Those who move among the people with their eyes open will not doubt that the number of non‐smokers is increasing, but mostly among older adults. Sales of cigarettes, despite the ban on advertising and the grim warning printed on packets, do not reflect this however, which can only mean that those who still smoke are the heavy smokers. This is a bad sign; as is the fact that youngsters, including a high percentage of those at school, openly flaunt the habit. The offence of using tobacco or any other smoking mixture or snuff while handling food or in any food room in which there is open food (Reg. 10(e)), remains one of the common causes of prosecutions under the Food Hygiene Regulations; it has not diminished over the years. The commonest offenders are men and especially those in the butchery trade, fishmongers and stall‐holders, but, here again, to those who move around, the habit seems fairely widespread. Parts of cigarettes continue to be a common finding especially in bread and flour confectionery, but also in fresh meat, indicating that an offence has been committed, and only a few of the offenders end up in court. Our purpose in returning to the subject of smoking, however, is not to relate it to food hygiene but to discuss measures of control being suggested by the Government now that advertising bans and printed health warnings have patently failed to achieve their object.

Abstract

Purpose

To take a more holistic and integrated view than in existing studies of export capability among small and medium‐sized enterprises, by exploring the key components of marketing management, and the blend of processes, practices and activities most closely associated with high levels of overall export performance.

Design/methodology/approach

The principal research instrument is the export marketing profiling system developed by the authors over a five‐year period, which provided the framework for data collection and analysis. Findings are derived from 250 semi‐structured interviews conducted in SMEs in the Yorkshire and Humber region of the UK.

Findings

The study identified 17 key practices, processes and activities that, taken together, are closely associated with export performance. They relate primarily to export marketing strategy, and suggest that a blend of capabilities in the areas of knowledge management (including market research and marketing intelligence), relationship‐building, product strategy and pricing are most closely associated with success.

Practical implications

The study has particular implications for business‐support providers. The findings could be used to assess and evaluate export capability in a wider sense. The conceptual framework could serve as a diagnostic tool for the identification of the areas of operation in which support, intervention and investment might have the greatest impact on overall export capability. Further research in these areas would be of particular importance in the drive to understand the relationship between export capability and performance.

Originality/value

This study's holistic approach to identifying the blend of capabilities most closely associated with high levels of export performance could inform national and local government policy in respect of the role of support agencies in improving the export performance of SMEs, in the UK or elsewhere. It offers a template for further research into key practices, processes and activities.

Abstract

Purpose

The purpose of this paper is to detail the rationale for, and development of, the Export Marketing Profiling System; a methodology for profiling and benchmarking the capabilities of small and medium‐sized enterprises (SMEs) to compete internationally.

Design/methodology/approach

It is the outcome of four years of research, incorporating quantitative, qualitative and longitudinal studies into the international activities of SMEs.

Findings

The findings of this research are utilised in the development of a system that recognises key themes and distils them into the specific characteristics, competencies, capabilities and activities that contribute to successful performance in international markets.

Originality/value

The value of this system lies in the population of a large database of SMEs that have been profiled using in‐depth interviews. This enables users to profile and benchmark a company's export capabilities against other SMEs. It has been piloted successfully in the Yorkshire and The Humber region as part of the Regional International Trade Strategy and has also been disseminated to business support agencies in the UK and Europe, where it is currently used to identify support needs and evaluate the impact of specific interventions over time.

Abstract

The information which has hitherto appeared in the daily press as to the evidence laid before the Departmental Committee which is inquiring into the use of preservatives and colouring matters can hardly have afforded pleasant reading to the apologists for the drugging of foods. It is plainly the intention of the Committee to make a thorough investigation of the whole subject, and the main conclusions which, in the result, must bo forced upon unbiassed persons by an investigation of this character will be tolerably obvious to those who have given serious attention to the subject. At a later stage of the inquiry we shall publish a full account of the evidence submitted and of the Committee's proceedings. At present we may observe that the facts which have been brought forward fully confirm the statements made from time to time upon these matters in the BRITISH FOOD JOURNAL, and amply justify the attitude which we have adopted on the whole question. Representatives of various trade interests have given evidence which has served to show the extent to which the practices now being inquired into are followed. Strong medical evidence, as to the dangers which must attach to the promiscuous and unacknowledged drugging of the public by more or less ignorant persons, has been given; and some medical evidence of that apologetic order to which the public have of late become accustomed, and which we, at any rate, regard as particularly feeble, has also been put forward. Much more will no doubt be said, but those who have borne the heat and burden of the day in forcing these matters upon the attention of the Legislature and of the public can view with satisfaction the result already attained. Full and free investigation must produce its educational effect ; and whatever legal machinery may be devised to put some kind of check upon these most dangerous forms of adulteration, the demand of the public will be for undrugged food, and for a guarantee of sufficient authority to ensure that the demand is met.

At the passing of the Fair Trading Act, 1973, and the setting up of a Consumer Protection Service with an Office of Fair Trading under a Director‐General, few could have visualized this comprehensive machinery devised to protect the mainly economic interests of consumers could be used to further the efforts of local enforcement officers and authorities in the field of purity and quality control of food and of food hygiene in particular. This, however, is precisely the effect of a recent initiative under Sect. 34 of the Act, reported elsewhere in the BFJ, taken by the Director‐General in securing from a company operating a large group of restaurants a written undertaking, as prescribed by the Section, that it would improve its standards of hygiene; the company had ten convictions for hygiene contraventions over a period of six years.

Government appointed and sponsored committees of every description—select, ad hoc, advisory, inquiry—such a prominent feature of the public scene since the last War, are understandable, even acceptable, reflect the urgency of the times in which we live. In the gathering gloom of more recent twilight years, they have flourished inordinately, especially in the socio‐political field, where most of their researches have been conducted. Usually embellished with the name of the figure‐head chairman, almost always expensively financed, they have one thing in common—an enormous output of words, telling us much of what we already know. So much of it seems dull, meaningless jargon, reflecting attitudes rather than sound, general principles.
